Fees
Per
Session

 The
fee is $200.00 for a 45 minute session, $50.00
for group session.
Upon
request, I
will
provide
a
bill
for
paid
services
which
you
may
submit
to
your
insurance
company
for
reimbursement.
Some
companies
which
have
reimbursed
for
my
services
include
Aetna,
Cigna,
Horizon,
Oxford,
Empire
Blue
Cross
Blue
Shield,
and
UnitedHealthcare.
Since
coverage
varies
widely,
it
is
recommended
that
you
contact
your
insurance
company
to
determine
which
services
your
policy
includes.
